SMS Sign up Form Requirements

All companies that use TeleGo SMS must have a Contact Us form on their website. You can request any other information, but the form must have the following fields for SMS applicants to submit:

Name
Phone Number

There must also be a check box at the end of the form. Next to the checkbox there must be the following text:
