Therapy Program

Physical therapy, occupational therapy, and speech / language pathology services provided through the agency will:

  • Analyze and interpret patient’s needs on the basis of the medical history, pertinent precautions, limitations and evaluative findings
  • Identify short and long-term goals that are measurable, objective and related to functional mobility as determined by medical history and evaluative findings
  • Formulate a treatment plan to achieve the goals identified
  • Review physician’s request for treatment, and if appropriate, recommend revisions on the basis of evaluative findings

This Medicaid program provides services to chronically ill or physically disabled individuals who have a medical need for help with activities of daily living (ADLs) or skilled nursing services.

  • Services can include any of the services provided by a personal care aide (home attendant), home health aide, or nurse.
  • Recipients have flexibility and freedom in choosing their caregivers.
  • The consumer or the person acting on the consumer’s behalf (such as the parent of a disabled or chronically ill child) assumes full responsibility for hiring, training, supervising, and – if need be – terminating the employment of persons providing the services

  • Diabetic care and nutrition counseling
  • Wound care
  • Medication management and teaching including injections
  • Colostomy care
  • Assesses patient health status, makes recommendation and referrals for PT/QT/SPT/Social workers and other medical providers
  • Assist physician in the evaluation of a patient’s level of function
